Our Community Health Worker Program
To address SCD disparities in Philadelphia and the Delaware Valley, we established a Community Health Worker program in 2020. Our Community Health Workers (CHWs) are passionate advocates for individuals living with SCD to ensure they receive the highest level of compassionate healthcare and social services. In addition to providing support to patients and their families, they conduct research and outreach, offer health education, and case management.

Crescent Cares is thrilled to address healthcare and maternal health disparities that affect the Sickle Cell community disproportionately through our new Equitable Access To Rejuvenating Therapeutic Healthcare (E.A.R.T.H.) grant. The E.A.R.T.H. program will help minimize barriers to comprehensive, culturally sensitive and holistic providers, therapies and resources that support the quality of life for Sickle Cell Warriors by making doula services, midwifery, therapy, acupuncture and massage therapy accessible.
