Our Community Health Worker Program

Crescent Cares

To address SCD disparities in Philadelphia and the Delaware Valley, we established a Community Health Worker program in 2020. Our Community Health Workers (CHWs) are passionate advocates for individuals living with SCD to ensure they receive the highest level of compassionate healthcare and social services. In addition to providing support to patients and their families, they conduct research and outreach, offer health education, and case management.

WHAT WE OFFER

A growing body of evidence-based practices demonstrates that implementing a CHW program is a solution that delivers meaningful and measurable results to hospitals and health systems. Research has shown that CHW programs have resulted in an average savings of $2,245 per patient served, and hospitals and health systems can save $2.28 for every $1.00 it invests in a CHW program – all while improving the health outcomes of patients and communities.
Advocacy for individuals living and community needs.
Assistance in navigating the health and human services systems.
We aim to bridge the gap between the SCD community and the health and social services systems.
Direct services such as trainings and educational services, at-home visitations, phone calls, and additional outreach.
Build individual and community capacity.

E.A.R.T.H Program

Crescent Cares is thrilled to address healthcare and maternal health disparities that affect the Sickle Cell community disproportionately through our new Equitable Access To Rejuvenating Therapeutic Healthcare (E.A.R.T.H.) grant. The E.A.R.T.H. program will help minimize barriers to comprehensive, culturally sensitive and holistic providers, therapies and resources that support the quality of life for Sickle Cell Warriors by making doula services, midwifery, therapy, acupuncture and massage therapy accessible.
