CRESCENT CARES
To address SCD disparities in Philadelphia and the Delaware Valley, we established a Community Health Worker program in 2020. Our Community Health Workers (CHWs) are passionate advocates for individuals living with SCD to ensure they receive the highest level of compassionate healthcare and social services. In addition to providing support to patients and their families they conduct research and outreach, make home visits, offer health education, and case management.
